David van Moolenbroek 3d697930dd TTY: use libchardriver; clean up
- writing to a PTY master side blocks if there is not already a
  blocked reader on the slave side, and select now reflects this;
- internally, TTY now uses a test based on "caller != NONE" rather
  than "grant != GRANT_INVALID" to identify whether a call is
  currently ongoing;
- "offset" fields have been removed as they equal the corresponding
  "cum" fields;
- improved variable typing and function naming here and there;
- various other small fixes.

Gianluca Guida fa59fc6eb4 Move shared headers in common/include
Headers that will be shared between old includes and NetBSD-like includes
are moved into common/include tree. They are still copied in /usr/include
in 'make includes', so compilation and programs aren't be affected.

Ben Gras 82173212ee Introduction of a 'big' ioctl, with more bits (20) reserved for encoding the
size field. The TIOCSFON ioctl size (8192) didn't get encoded properly,
as there weren't enough bits for it (12) in the regular format.

The new format has only one type field, and an extra flag (_IOC_BIG)
turned on. FS checks for this flag and uses the alternative decoding
of the ioctl codes to determine the size when doing grants.

This unbreaks loadfont, although that still uses a phys copy in tty.
