SLICC uses pointers for cache and TBE entries but not for directory entries.
This patch changes the protocols, SLICC and Ruby memory system so that even
directory entries are referenced using pointers.

In addition to obvious changes, this required a slight change to the slicc
grammar to allow types with :: in them. Otherwise slicc barfs on std::string
which we need for the headers that slicc generates.
